Includes a doctor-patient interview (history taking), physical assessment, report of findings (that includes informing you of your diagnosis and proposed plan of management) and a treatment (if possible).
Subsequent Treatments – $50.00
Custom Orthotics – $450.00
Includes an assessment of the patient, the casting, dispensing, sizing and any modifications needed to the orthotics, and all paper work required for the process.
